Idiopathic Parkinson’s disease impairs working memory, but the exact nature of this deficit in terms of the underlying cognitive mechanisms is not well understood. In this study patients with mild clinical symptoms of PD were compared with matched healthy control subjects on a computerized battery of tests designed to assess spatial working memory and verbal working memory. In the spatial working memory task, subjects were required to recall a sequence of four locations. The verbal working memory task was methodologically identical (...) except for the modality of the stimuli used, requiring subjects to orally recall a sequence of six digits. In either case, half of the sequences were structured in a way that allowed ‘chunking’, while others were unstructured. This manipulation was designed to dissociate the strategic component of task performance from the memory-load component. Mild medicated patients with PD were impaired only on the structured versions of the verbal working memory tasks. The analogous deficit in the spatial working memory was less pronounced. These findings are in agreement with the hypothesis that working memory deficits in PD reflect mainly the executive component of the tasks and that the deficits may be at least partly modality-independent. Rogers suggests that human spirituality comprises the interplay of three foundational human dynamics: suffering, interconnecting, and valuing. The spiritual orientations that result from these clusters of dynamics are primarily integrating or disintegrating. The dominant spiritual orientation in this country has been disintegrating; and this orientation has characterized the attitude of the majority population toward African Americans since the beginning of slavery, fostering radical separation, displacing the suffering of the majority onto a minority, and defining the experience and perspectives of (...) the majority as normative without regard for the minority. Rogers explores the effects of this disintegrating spiritual orientation in terms of the dynamics of oppression using the ideas of 'being' and 'non-being'. The key to ameliorating oppression is to recognize the liberating capacity of suffering as potentially subversive within ever-enlarging contexts addicted to increasing material consumption and sensual gratification. ;Chapter 1 is a general introduction to the dissertation. ;Chapter 2 provides an overview of the primary methodology for research in 'formation theory' as developed by Adrian van Kaam. Using issues from the philosophy of science, the chapter also discusses the basis for divergence from the standard methodology based in the anomalous character of the researcher's 'formative event' and perspective relative to concerns that have been researched using formation theory. ;Chapter 3 considers a conceptualization of spirit as questing for meaningfulness, which provides the experiential ground for relating spirituality and liberation: Since oppression precipitates meaninglessness through disintegration, the reaffirmation of an integrative spirituality provides the ground for meaningfulness, which in turn grounds liberation, or transcendence. ;Chapter 4 presents the construct of human spirituality using the three foundational foci of spiritual formation: suffering, relating, and valuing. ;Chapter 5 concludes the dissertation using the two principle laws of thermodynamics to develop an analogous understanding for the necessity of suffering in maintaining an integrative and liberating spirituality. Dr Adrian Rogers delivers a three-pronged assault on the concepts of 'situational ethics'; the eleemosynary principles underlying the National Health Service, and the permissiveness of modern society. I hold strong, and largely opposite, views on each of these matters, which are outlined in this paper.

Americans have always been divided over whether to welcome or to discourage immigration. But virtually all American leaders have rested their views on notions that the United States has unique providential or world-historical significance-as an asylum for the world's oppressed, as a model to the world, or even as the world's leader. Today, it is normatively desirable for the U.S. to view itself not as the world's "city on a hill" but simply as one worthy political society among many others. (...) Populist protests against Coronavirus-related restrictions in the US appear paradoxical in three respects. Populism is generally hostile to expertise, yet it has flourished at a moment when expertise has seemed more indispensable than ever. Populism thrives on crisis and indeed often depends on fabricating a sense of crisis, yet it has accused mainstream politicians and media of overblowing and even inventing the Corona crisis. Populism, finally, is ordinarily protectionist, yet it has turned anti-protectionist during the pandemic and challenged the allegedly (...) overprotective restrictions of the nanny-state. I address each apparent paradox in turn before speculating in conclusion about how populist distrust of expertise, antipathy to government regulation, and skepticism toward elite overprotectiveness may come together – in the context of intersecting medical, economic, political, and epistemic crises – in a potent and potentially dangerous mix. This was the way in which the living God of biblical tradition was described by the great Medieval philosophers such as Augustine, Anselm and Aquinas.Contemporary philosophers find much to question, criticise and reject in the traditional analysis of that description. Some hold that the attributes traditionally ascribed to God - simplicity, necessity, immutability, eternity, omniscience, omnipotence, creativity and goodness - are inherently incoherent individually, or mutually inconsistent. In this prcis we focus on phenomena central to the reaction against similarity-based theories that arose in the 1980s and that subsequently motivated the approach to semantic knowledge. Specifically, we consider (1) how concepts differentiate in early development, (2) why some groupings of items seem to form or coherent categories while others do not, (3) why different properties seem central or important to different concepts, (4) why children and adults sometimes attest to beliefs that seem to contradict their direct experience, (...) (5) how concepts reorganize between the ages of 4 and 10, and (6) the relationship between causal knowledge and semantic knowledge. The explanations our theory offers for these phenomena are illustrated with reference to a simple feed-forward connectionist model. The relationships between this simple model, the broader theory, and more general issues in cognitive science are discussed. (shrink)

Context Professionalism remains a challenging part of the medical curriculum to define, teach and evaluate. We suggest that one way to meet these challenges is to clarify the definition of professionalism and distinguish this from medical ethics. Methods Our analysis is two staged. First, we reviewed influential definitions of professionalism and separated elements relating to (a) ethico-legal competencies, (b) clinical competence and (c) professionalism. In reference to professionalism, we then distinguished between aspirational virtues/values and specific behaviours. From these, we develop (...) a working definition of medical professional behaviour consisting of six domains of behaviour: responsibility; relationships with and respect for patients; probity and honesty; self awareness and capacity for reflection; collaboration and team work; and care of colleagues. Second, we tested this working definition against empirical data concerning disciplinary action against practising doctors using (a) sources in the literature and (b) an original analysis of complaints received by the Medical Board of South Australia. Conclusion Our empirical analysis supports the relevance of four of the six potential domains: responsibility; relationships with and respect for patients; probity and honesty; self awareness and capacity for reflection. There are additional reasons for retaining ‘collaboration and team work’ in the medical professional behaviour curriculum but ‘care of colleagues’ may be better addressed in the ethico-legal curriculum. Our definition of professional behaviour is consistent with the theoretical literature, captures behaviours that predict future complaints against practitioners and is consistent with current complaints about professionalism in South Australian practitioners. This definition can further the teaching and assessing of professional behaviour in medical schools. (shrink)

Rogers canvases the literature critical of recent experiments, adding new criticisms of her own. She argues these experiments should not undermine belief in human freedom and lists ethical and practical problems facing the attempt to study free will experimentally.

Overdiagnosis refers to diagnosis that does not benefit patients because the diagnosed condition is not a harmful disease in those individuals. Overdiagnosis has been identified as a problem in cancer screening, diseases such as chronic kidney disease and diabetes, and a range of mental illnesses including depression and attention deficit hyperactivity disorder. In this paper, we describe overdiagnosis, investigate reasons why it occurs, and propose two different types. Misclassification overdiagnosis arises because the diagnostic threshold for the disease in question has (...) been set at a level where many people without harmful disease are nonetheless diagnosed. We illustrate misclassification overdiagnosis using the example of chronic kidney disease. Misclassification occurs in diseases diagnosed using biomarkers or based on patient reported phenomena. Maldetection overdiagnosis arises because, at the time the diagnosis is made and despite the presence of a 'gold standard' diagnostic test, it is not possible to discriminate between harmful and non-harmful cases of the index disease. We illustrate maldetection overdiagnosis using the example of thyroid cancer. While there is some overlap between misclassification and maldetection overdiagnosis, this conceptual analysis helps to clarify the phenomenon of overdiagnosis and is a necessary first step in developing strategies to address the problem. In this paper I argue that it is morally important for doctors to trust patients. Doctors' trust of patients lays the foundation for medical relationships which support the exercise of patient autonomy, and which lead to an enriched understanding of patients' interests. Despite the moral and practical desirability of trust, distrust may occur for reasons relating to the nature of medicine, and the social and cultural context within which medical care is provided. Whilst it may not be possible to trust (...) at will, the conscious adoption of a trusting stance is both possible and warranted as the burdens of misplaced trust fall more heavily upon patients than doctors. Human beings naturally care a great deal for themselves--and couldn't survive otherwise. As Aquinas observed, the drive for self-preservation is the first law of nature. Yet in the imperative of self-love, philosophers have also perceived a tacit threat. Plato reminds us that 'the excessive love of self is in reality the source to each man of all offences.' And so the inevitability of self- concern must be balanced with its manifest potential for harm. But how is such a reconciliation possible? (...) This collection brings tohether the efforts of twenty- three great thinkers addressing such themes as the nature of self-interest, its connection to benevolence and morality, and its implications for political theory. In this series of essays, the author sets out the traditional, Anselmian views on certain questions in the philosophy of religion, and aims to defend these views in the contemporary idiom.

This paper argues that, in addition to humanitarian concerns, policies toward immigrants should also be shaped by recognition of special responsibilities toward some populations of noncitizens. National governments acquire such responsibilities in part through their histories of coercive impositions on those populations. Former imperial powers, in particular, often possess special obligations toward the inhabitants of their foreign colonies that go beyond their general humanitarian responsibilities. Those obligations might be met in various ways; but if national governments of wealthy, formerly imperial (...) states granted greater access for immigration to the poor of their former colonies, migration flows might significantly contribute to reductions in global levels of inequality. Bioethics is a practically oriented discipline that developed to address pressing ethical issues arising from developments in the life sciences. Given this inherent practical bent, some form of advocacy or activism seems inherent to the nature of bioethics. However, there are potential tensions between being a bioethics activist, and academic ideals. In academic bioethics, scholarship involves reflection, rigour and the embrace of complexity and uncertainty. These values of scholarship seem to be in tension with being an activist, which requires pragmatism, (...) simplicity, certainty and, above all, action. In this paper I explore this apparent dichotomy, using the case example of my own involvement in international efforts to end forced organ harvesting from prisoners of conscience in China. I conclude that these tensions can be managed and that academic bioethics requires a willingness to be activist. (shrink)

Marriagelike homosexual relationships expose a division among ethicists following Aquinas. Those emphasizing natural law may call such relationships unnatural; those emphasizing the virtues may approve of relationships fostering love and justice. Natural law, the virtues, and homosexuality all show up in Aquinas's "Commentary on Romans"--untranslated and hardly cited. Romans 1:18 opens a discussion of justice. Verse 20 provides Aquinas's chief warrant for natural law. Verse 26 applies virtue and law to "the vice against nature." But Aquinas's account also depends on (...) Paul as an exemplar of virtue and on Aquinas's high regard for the Bible. Aquinas deploys natural law as a mode of biblical exegesis, not an alternative to it. In the "De potentia", Aquinas considers how to proceed when nature and Scripture seem to conflict. The account does not settle, but rather makes more room for, dispute. (shrink)

In this paper I shall argue that to a very significant extent mathematics is concept analysis, and that though the analysis of mathematical concepts is in a number of ways different from the analysis of philosophic concepts, the similarities between these two types of concept analyses are as important and far reaching as the differences. I shall argue that because mathematics and philosophy are each concerned with the analysis of concepts, they are much more like one another epistemologically than is (...) often recognized. In insisting upon fundamental similarities between mathematics and philosophy, I shall be agreeing with the classical rationalists, but on a very different conception of both philosophy and mathematics from that held by the rationalists. The rationalists wished to assimilate philosophy to mathematics as understood in their time; viz. as a body of necessary propositions, which followed from self-evident axioms and postulates, revealed to the natural light of reason. As against this rationalistic position, I wish to make a comparison in the reverse direction, in which I shall presuppose a certain conception of philosophy as something given, and then insist that mathematics is in many important respects similar to philosophy as so understood. In particular, I wish to insist that there is a significant comparison between mathematics on the one hand, and philosophy as understood by probably a majority of philosophers today on the other--viz., philosophy understood as concept analysis--, where it is conceded that the analysis of philosophic concepts is inherently a tentative matter, wherein it is impossible--at least in the usual case--to offer any one analysis of a given philosophic concept as absolutely certain and beyond all revision. I shall argue that by virtue of the fact that mathematics, like philosophy, is concerned with the analysis of concepts, many at least of the propositions advanced within it are inherently revisable, and do not possess the kind of certainty the rationalists ascribed to them.
